The first step in the application process is to contact our Intake Coordinator by calling (617)-348-6756 to schedule an appointment for an Open House. During the Open House, the student and guardian (and/or caseworker) will have the opportunity to ask any and all questions as well as complete the necessary intake documents.
Should the student like to continue the intake process s/he will complete the written application and will participate in two interviews, one with the Recovery Counselor and one with the Guidance Counselor. There may be a third interview if the student has an IEP or 504 plan. Following the interviews will be two assessments geared around education and trauma. These interviews and assessments may be scheduled upon completion of the Open House.
The following documentation is required by the interview date:
- Most recent psycho-social assessment (clinical evaluation)
- All high school academic transcripts
- Special education plan (e.g. IEP, 504 plan) if applicable
- MCAS scores/SAT scores if applicable
- Health Insurance Card (front & back)*
- Birth Certificate*
- Social Security Card*
- Immunization Records
- Most Recent Physical
